Tube type, size, speaking valve use and change schedules are set by the patient's specialist team or tracheostomy nurse — always replace like with like unless advised otherwise. Keep a spare tube (and one a size smaller) and emergency equipment with the patient at all times, as your care plan directs. Important: a speaking valve must only be used with a deflated cuff or an uncuffed tube, and only when your clinical team has assessed it as safe. Seek urgent help if a person with a tracheostomy has difficulty breathing or the tube becomes blocked or dislodged.
